How do I complete my new patient paperwork?
Your new patient paperwork can be found by logging into the therapy portal
After scheduling your initial appointment, you will receive an email to set up a therapy portal account. The link to set up the portal expires in 7 days. If you experience any trouble accessing the therapy portal you can call the office to speak with Heather or email her and she can send you a link to reset your password. If you call outside of her working hours, leave a message and she can return your call.

Please make sure you have Adobe Acrobat Reader DC (the free version) if you would like to electronically fill out these documents. You will not be able to sign the documents if you complete them in your browser on your computer or your phone. You must open up the document in the program to sign- you can download Adobe Acrobat Reader DC for free - Or, if needed, you may print the paperwork out, fill it out manually and bring it with you to your 1st appointment.
**It is critical that you complete your new patient paperwork for your clinician in time for your 1st appointment.
How do I pay for my/my child’s appointments?
We require that a payment method be kept on file so that your clinician may process payment at the completion of you/your child’s appointment. If you choose to use an HSA card for payment we require a backup credit card to be kept on file as well. All the clinicians at BABH do their own billing. Typically, billing is completed within 1-2 weeks of the appointment.
How do I schedule my/my child’s next appointment?
There are a couple ways that you can do this:
- You can schedule with your clinician at the end of your last appointment.
- You can log in to the therapy portal and request an appointment that is most convenient for you, based upon your clinician’s availability.
- You can call the office to speak with Heather. If you do not reach her within her office hours, leave a message and she will return your call.
How do I cancel my/my child’s appointment?
As our cancellation policy states, we ask for at least 24 hours notice when you need to cancel or reschedule an appointment. This allows us to offer that newly opened time to patients who are waiting for an appointment. We realize that there may be extenuating circumstances that would not allow for this. In those cases, we ask you to cancel your appointment as soon as you know you will be unable to attend. Per our policy, you may be charged for appointments missed or canceled without 24 hours notice.
- You are able to cancel appointments that are 24+ hours away from the appointment time through the therapy portal.
- If your cancellation falls within that 24 hour window of your appointment you will need to reach out to your clinician directly and/or call Heather.
How do I change my payment method on file?
Payment method may be updated by informing your clinician or contacting Heather. A credit card authorization form can be sent to you through the therapy portal or you can call Heather to update your payment method over the phone.
How do I contact my/my child’s clinician?
Our clinicians are often unable to answer calls throughout the day when they are seeing patients. It may be best to contact your clinician through email, as long as you are aware of the potential risks of using email to communicate as described in the service agreement and consent form. Your clinician may be able to respond to your question via email or they may suggest scheduling a phone call or appointment if required. Parents are also encouraged to ask the clinician for a portion of their child’s scheduled therapy appointment to discuss updates, concerns, or questions as needed.

How can I get a receipt that I can submit to my insurance company for reimbursement?
We are more than happy to provide a receipt for your services at BABH. Contact either your child’s clinician or Heather to receive a receipt.
